If you have a long term condition like type 2 diabetes, a heart condition, high blood pressure, kidney disease or have had a stroke you will be invited to attend a Long Term Condition Clinic once each year. This will mean that you will have an initial appointment with the Health Care Assistant, who will perform all of the pre-check tests, followed by an appointment with the Practice Nurse a week later. The nurse will be able to update your medication and ensure that your condition is well managed.
